Camper Registration
If you are a burn survivor who is planning to attend Camp Susquehanna there are a number of forms that you will need to fill out and send to Marcia Levinson. The forms that you will need to give to Marcia can be found within a link on the left side of this page. You will only be able to access these documents if you have been given the password. A brief description of each form can be found below.

Forms need to be filled out by a parent or guardian. You will be told which forms to download and fill out, not all activities are done every year.

Forms should not be returned until May. However, please send an e-mail of interest or send a letter to Marcia Levinson prior to May or A.S.A.P. so that we can add you to the mailing list.

Camper Application
This form is required if you plan to attend Camp Susquehanna. In this application you will be asked a number of questions about the camper planning to attend, and general information on how we can contact you if necessary.

Adventure Challenge Experience Application
If you take a look at the form by clicking on the left, you will find a brief explanation of what the Adventure Challenge Experience is. The Adventure Challenge Experience is optional, however we encourage campers to sign up for it.

Scuba Diving Instructions
At Camp Susquehanna we offer scuba diving instructions to campers if they want to learn how to scuba dive. If your child would like to learn this, please look at all four of the forms that are on the lefthand side of the page. All of the information must be filled out on these unless otherwise noted. Please turn these forms in with your applications and all other information.

Media Release Form
This is only necessary if you want to allow Camp Susquehanna to use videos, photos, or other forms of media that have your child in them. If you would not like your child to be in any of these forms of media, you are not required to hand this form in.
